چکیده

Aim: To report the clinical courses of two patients, one with Hodgkin’s lymphoma (HL) and Non-Hodgkin’s (NHL), who developed severe refractory acute respiratory distress syndrome (ARDS) were treated veno-venous extracorporeal membrane oxygenation (VV ECMO). Case report: Both patients chemotherapy-associated febrile neutropenia followed by pneumonia ARDS, after which they transferred to intensive care unit. Their failure deteriorated despite endotracheal intubation protective mechanical ventilation, at point a decision for VV ECMO initiation was made. had complicated treatment ECMO-associated complications. The most important complications support in our HL patient cardiac arrest; right atrial laceration pericardial tamponade needed surgical treatment; leg ischemia required transfemoral amputation; thrombosis within oxygenator; several septic episodes hemodynamic instability; sided tension pneumothorax. Despite all difficulties, successfully weaned from ECMO. Unfortunately, he died prior hospital discharge as result sepsis multiple organ failure. significant ECMO-induced NHL bleeding incidents, notably diffuse oropharyngeal continuous bilateral pulmonary hemorrhage; superimposed bacterial pneumonia; extensive pneumomediastinum subcutaneous emphysema. therapeutic efforts, during because decompensation. Conclusions: hematologic malignancies (HMs) undergoing have poor outcomes, high rates Further studies focusing on selection issues concerning prevention, diagnosis are needed.

Veno–veno–arterial extracorporeal membrane oxygenation treatment in patients with severe acute respiratory distress syndrome and septic shock

In acute respiratory distress syndrome (ARDS) with refractory septic shock, isolated veno–venous (VV) or veno–arterial (VA) extracorporeal membrane oxygenation (ECMO) may lead to differential hypoxia or inadequate tissue perfusion [1]. In this context, MacLaren et al. [2] showed that central ECMO improved the outcomes by guaranteeing systemic oxygenation without differential hypoxia. However, c...

Veno-venous extracorporeal membrane oxygenation in a patient with severe acute respiratory failure – case report

Acute respiratory failure resistant to conventional pulmonary therapy often requires intensive medical care. In rare cases, ventilator therapy proves insufficient, and only the option of employing veno-venous extracorporeal membrane oxygenation (ECMO V-V) remains. The present article describes the case of a 23-year-old patient who experienced severe acute respiratory distress syndrome with asso...

Improvement of Oxygenation in Severe Acute Respiratory Distress Syndrome With High-Volume Continuous Veno-venous Hemofiltration

The efficacy and therapeutic mechanisms of continuous renal replacement therapy (CRRT) for improvement of oxygenation in acute respiratory distress syndrome (ARDS) remain controversial. These questions were addressed by retrospective analysis of severe ARDS patients admitted to the pediatric intensive care unit of our hospital from 2009 to 2015 who received high-volume continuous veno-venous he...

Efficacy of Veno-Venous Extracorporeal Membrane Oxygenation in Severe Acute Respiratory Failure

PURPOSE The objective of this study was to evaluate our institutional experience with veno-venous (VV) extracorporeal membrane oxygenation (ECMO) in patients with severe acute respiratory failure (ARF). MATERIALS AND METHODS From January 2007 to August 2013, 31 patients with severe ARF that was due to various causes and refractory to mechanical ventilation with conventional therapy were suppo...

Severe hypoxemia during veno-venous extracorporeal membrane oxygenation: exploring the limits of extracorporeal respiratory support

OBJECTIVE Veno-venous extracorporeal oxygenation for respiratory support has emerged as a rescue alternative for patients with hypoxemia. However, in some patients with more severe lung injury, extracorporeal support fails to restore arterial oxygenation. Based on four clinical vignettes, the aims of this article were to describe the pathophysiology of this concerning problem and to discuss pos...
